Substance Abuse Program Requirements

STABILITY - ACHIEVEMENT - PROGRESS

I. Treatment Phases

I. PHASE 1 (two weeks) - $150.00/two weeks ($15.00 per meeting)
1.1 Participant must attend substance abuse treatment group meetings daily each week.
1.2 Weekly urinalysis at random and as requested by CSO.
1.3 Participant must pass a minimum of two (2) consecutive UA's in order to progress to Phase 2.
1.4 Participant must strictly comply with all conditions of community supervision.
1.5 CCL4 Progress Hearing with CSO @ Compliance Docket.

II. PHASE 2 (two weeks) - $90.00/two weeks ($15.00 per meeting)
2.1 Participant must attend substance abuse treatment group meetings three (3) times each week and as requested by CSO.
2.2 Weekly urinalysis at random and as requested by CSO.
2.3 Participant must pass a minimum of two (2) consecutive UA's in order to progress to Phase 3.
2.4 Participant must strictly comply with all conditions of community supervision.
2.5 CCL4 Progress Hearing with CSO @ Compliance Docket.

III. PHASE 3 (eight weeks) $240.00/eight weeks ($15.00 per meeting)
3.1 Participant must attend substance abuse treatment group meetings two (2) times each week and as requested by CSO.
3.2 Three (3) random urinalysis and as requested by CSO.
3.3 Participant must pass a minimum of two (2) consecutive UA's in order to progress to Phase 4.
3.4 Participant must attend AA/NA/Faith Based Program or a cognitive based/life skills program approved by the Court or CSO once a week.
3.5 Participant must strictly comply with all conditions of community supervision.
3.6 CCL4 Progress Hearing with CSO @ Compliance Docket.

IV. PHASE 4 -AFTERCARE (twelve weeks) - $180.00/twelve weeks ($15.00 per meeting)
4.1 Participant must attend substance abuse treatment group meetings once each week and as requested by CSO.
4.2 Bi-monthly urinalysis and as requested by CSO.
4.3 Participant must pass a minimum of six (6) consecutive UA's in order to progress to Phase 5.
4.4 Participant must attend AA/NA/Faith Based Program or a cognitive based/life skills program approved by the Court or CSO once a week.
4.5 Participant must strictly comply with all conditions of community supervision.
4.6 CCL4 Progress Hearing with CSO @ Compliance Docket.

V. PHASE 5-SELF-SUPPORT (20 Weeks) - $300.00 ($15.00 per meeting)
5.1 Participant must attend the self-support group session (relapse prevention group) once each week and any additional ones requested by CSO.
5.2 Random urinalysis once every four weeks and as requested by CSO.
5.3 Participant must pass a minimum of six (6) consecutive UA's in order to complete the program.
5.4 Participant must attend AA/NA/Faith Based Program or a cognitive based/life skills program approved by the Court or CSO once a week.
5.5 Participant must strictly comply with all conditions of community supervision.
5.6 CCL4 Progress Hearing with CSO @ Compliance Docket

II. General Guidelines

A. Participant agrees to waive a record on all progress hearings.
B. Participant agrees to submit his person, property, place of residence, vehicle, personal effects, to search at any time, with or without a search warrant, warrant of arrest by any probation officer or law enforcement officer.
C. Participant must maintain strict compliance with all Conditions of Community Supervision as set forth in participant's Judgment and Order Granting Community Supervision and any violation of the Order shall immediately be brought to the attention of the Court.
D. Participant agrees to be placed on Intensive Supervision Probation for a period of one year or as directed by the Court.
E. Participant must pass the minimum urinalysis requested by the community supervision department in order to progress to the next phase.

NOTE: A POSITIVE OR DILUTE URINALYSIS
WILL RESULT IN IMMEDIATE JAIL SANCTIONS


F. After graduation the Court may Order a Maintenance Protocol of ongoing group sessions over a 42 week period. ($15.00 per session)
G. The SAP Team has created an ongoing Self Help Support Group that any participant or Graduate may attend free of charge.
H. Any additional sessions ordered in Phases 3, 4 and 5 above those required in each Phase will be at no charge to the participant.
