Reminders Prior to Appointment:
1. Please make sure to have your DS-11 completed in black ink prior to your arrival. White out/corrections will not be accepted. Please ensure that your DS-11 is the most current version.
2. Please make sure you bring a passport photo, as photos are not taken onsite. Do not staple your photo to your application.
3. Please ensure that you bring accepted forms of payment: Personal check, money order, or cashier's check (made payable to the Dept. of State) to be submitted with your application; Cash or credit/debit card for the $35 service fee.
