The Montgomery County District Clerk’s Office is now
processing passports by appointment only.

you to the Passport Office Location:

1. DS-11 APPLICATION (two pages completed in black ink only & not signed)
2. DRIVER’S LICENSE (or other GOVERNMENTAL ID) (bring a copy, front and back).
3. ORIGINAL OR CERTIFIED BIRTH CERTIFICATE plus a copy.
4. ONE PASSPORT PICTURE – brought in from any place that takes passport photos.
5. $35 DISTRICT CLERK ACCEPTANCE FEE* for each application (Cash or Credit/Debit Card**)
6. U.S. DEPARTMENT OF STATE FEE (see costs below) INDIVIDUAL CHECK, MONEY ORDER OR

on behalf of the U.S. Department of State, July 1, 2004, pursuant to Title 22CFR Section 51.61 of the Code of Federal Regulations.
